DR4 or FR4 for a 400G link?

DR4 spreads four lanes across a parallel single-mode MPO trunk, which suits breakout to 4x100G. FR4 carries the full 400G over one duplex single-mode pair on LC, which drops into structured cabling that was never built for parallel trunks. Both are single-mode and neither substitutes for the other.

Will a 400G QSFP-DD module fit a QSFP112 or OSFP port?

No. QSFP-DD, OSFP and QSFP112 are different form factors with different electrical interfaces, even at the same 400G line rate. Confirm the exact cage on the host port first; the rate label alone never establishes that a module will seat and link.

Can a 400G port break out to 4x100G?

With a parallel PMD and a host that supports the lane mode, yes. The source port has to be configured for breakout and each 100G branch port must accept that rate. A duplex PMD such as FR4 or LR4 cannot break out regardless of host configuration, because the lanes are combined onto one fiber pair inside the module.
