Which 40G QSFP+ PMD matches the fiber I already have?

Multimode MPO trunks point to SR4 or eSR4; duplex single-mode LC points to LR4 for 10km or ER4 for 40km; a single multimode strand points to BiDi. The series covers roughly 100m to 40km, so the installed fiber and connector usually decide before distance does.

Will a 40G QSFP+ module work in a 100G QSFP28 port?

Frequently yes, but it is a host question rather than a module property. Many QSFP28 ports accept a QSFP+ module when configured for 40G, while the reverse never works. Confirm the supported port speeds and software version on the target platform before planning a mixed-rate deployment.

Can a 40G QSFP+ port break out to 4x10G?

Over a parallel MPO PMD or a breakout cable, and only when the source port is set to 4x10G mode and each branch port accepts 10G. A duplex PMD such as LR4 or ER4 combines its lanes inside the module onto one fiber pair and cannot be broken out.
