Should I choose OSFP or QSFP-DD for 800G?

The switch, NIC or accelerator decides it, not the optics. The two cages are not interchangeable, so confirm which one the target port provides before selecting a PMD. OSFP has more thermal headroom in dense front panels, while QSFP-DD is often chosen for backward compatibility with an existing QSFP installed base.

Can an 800G port break out to 2x400G or 8x100G?

Only when the host port is configured for that mode and every branch port accepts the resulting rate. Breakout is a host configuration plus a matching cable or trunk, not a property of the module alone. Confirm the supported lane modes on the target platform before planning the fan-out.

Which 800G PMD covers my link distance?

Roughly: multimode VR8 and SR8 for in-row runs around 100m, DR8 over parallel single-mode for 500m, and 2xFR4 over duplex single-mode for 2km.
