Which 100G PMD matches my distance and fiber?

The listed range spans 100m to 40km. In practice: SR4 over multimode MPO for in-row links, PSM4 over parallel single-mode for around 500m, CWDM4 over duplex single-mode for about 2km, LR4 for 10km and ER4 for 40km. Start from the fiber and connector already installed, then confirm the loss budget.

Is 200G QSFP56 compatible with a 100G QSFP28 port?

No. QSFP56 uses PAM4 electrical lanes while QSFP28 uses NRZ, so the electrical interface differs even though the cages look alike. Some platforms allow a QSFP28 module in a QSFP56 port but not the reverse; treat this as a per-platform question and confirm the supported port modes.

Can a 100G port break out to 4x25G?

Only over a parallel PMD with the source port configured for 4x25G breakout, and only if all four branch ports support 25G. FEC mode and software version also have to be confirmed on the target platform, because a 25G branch may require FEC that the host is not configured to enable.
